JourneyCare Hospice in Chicago is seeking a Hospice Registered Nurse (RN) Case Manager to provide compassionate, patient-centered care. This full-time position offers manageable caseloads with a Monday–Friday schedule and only one weekend every eight weeks.

Qualifications

• Graduate of an accredited RN program.
• Active Illinois RN license.
• Current CPR and First Aid certification.
• Hospice or home health experience preferred.

Responsibilities

• Manage a dedicated hospice caseload as the primary RN Case Manager.
• Perform patient assessments, admissions, and individualized care planning.
• Deliver skilled nursing care focused on comfort and dignity.
• Coordinate care with physicians and interdisciplinary team.
