JourneyCare Hospice in Chicago, IL is seeking a compassionate Hospice RN Case Manager to provide patient-centered end-of-life care, manage caseloads, and collaborate with an interdisciplinary team. The role offers a Monday–Friday schedule with one weekend every eight weeks, a strong support system, and a salary between $80,000 and $97,000 plus benefits.

Active IL RN license and CPR/First Aid are required; hospice or home health experience preferred.

Qualifications

• Graduate of an accredited RN program.
• Active Illinois RN license.
• Current CPR and First Aid certification.
• Hospice or home health experience preferred.

Responsibilities

• Manage a dedicated hospice caseload as the primary RN Case Manager.
• Perform patient assessments, admissions, and individualized care planning.
• Deliver skilled nursing care focused on comfort and dignity.
• Coordinate care with physicians and interdisciplinary team members.
• Conduct routine visits and adjust care plans as needed.
• Provide end-of-life support, including family education and pronouncement.
• Maintain timely, accurate documentation.
• Supervise Home Health Aides in compliance with regulations.
