Job summary

Generations Hospice in Fond du Lac, WI is seeking a compassionate Hospice RN Case Manager to provide expert, patient-centered care at a critical time in people’s lives. You’ll assess needs, develop care plans, administer medications, and partner with a collaborative, interdisciplinary team in a home-based setting.

Qualifications

• Graduate of an accredited nursing program with current licensure to practice in the state of operation.
• 1 year nursing experience required; 3+ years preferred.
• Hospice or similar setting experience preferred.
• Knowledge of hospice philosophy of care.
• Commitment to clinical and documentation excellence.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
