Generations Hospice in Wisconsin is seeking a compassionate Hospice RN Case Manager to provide expert, patient-centered care in a supportive, team-based environment.

Join our experienced interdisciplinary team and enjoy a 1:1 care approach, flexible scheduling options, and opportunities for professional growth. Monday–Friday hours with travel throughout the territory, covering Winnebago and Fond du Lac Counties.

Qualifications

• Graduate of an accredited nursing program with current RN licensure in the state of operation.
• 1 year nursing experience required; 3+ years preferred.
• Hospice or similar setting experience preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must have a reliable, insured vehicle and be a licensed driver per state requirements.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans aligned with patient wishes and goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patient’s family and caregivers.
• Collaborate with the interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
