Generations Hospice is seeking a dedicated Hospice RN Case Manager to join our team in Wisconsin. You will provide expert, patient-centered care at a critical time, with a focus on compassionate end-of-life support and collaborative care within an interdisciplinary team.

The role offers supportive leadership, lower patient-to-nurse ratios, and the chance to work with an internal triage team to extend after-hours coverage. Travel across Winnebago & Fond du Lac counties is required.

Qualifications

• Graduate of a nursing program with an active RN license in Wisconsin.
• Minimum 1 year of nursing experience; hospice/home health preferred.
• Commitment to compassionate, patient-centered end-of-life care.
• Ability to drive and maintain reliable transportation.

Responsibilities

• Assess and monitor physical, emotional, and psychosocial needs of patients.
• Develop and implement individualized hospice care plans.
• Deliver direct nursing care: medications, treatments, and interventions.
• Provide pain and symptom management and education to families.
• Collaborate with the interdisciplinary team and document timely
• Maintain accurate charting and participate in on-call rotations.
• Support after-hours triage and mentor staff as needed.
