Job summary

Generations Hospice in Fond du Lac, WI seeks a compassionate Hospice RN to provide patient-centered care in a PRN role. You will assess needs, develop care plans, administer treatments, and educate families while working with an interdisciplinary team.

Enjoy strong leadership, after-hours triage support, and streamlined documentation via Homecare Homebase EMR. Travel within the territory is required, with flexible scheduling and career stability.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile that is insured and in good working order.

Responsibilities

• Assess/ monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patients wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
