Generations Hospice in Wisconsin seeks a dedicated Hospice RN Case Manager to deliver patient-centered care in a supportive, team-based environment. You will assess needs, develop care plans, and provide direct nursing care, medications, and symptom management for patients at a critical time.

Qualifications

• Graduate of an accredited nursing school and current RN licensure in the operating state.
• One year nursing experience required; 3+ years preferred for advanced roles.
• Experience in hospice or similar setting preferred.
• Knowledge of hospice philosophy of care.
• Commitment to clinical excellence and thorough documentation.
• Must have a valid driver’s license and insured vehicle.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans aligned with patient wishes and goals.
• Direct nursing care: medications, treatments, interventions.
• Provide pain and symptom management.
• Educate and support patients’ families and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate, timely documentation.
• Participate in on-call rotation as required by local branch.
