Job summary

Generations Hospice in Fond du Lac, WI is seeking a compassionate Hospice RN (PRN) to deliver patient-centered care in home and facility settings. You’ll assess needs, create care plans, administer meds, and educate families. Work with a supportive interdisciplinary team and enjoy flexible scheduling with strong community impact.

Requirements include RN licensure in Wisconsin, 1 year nursing experience (3+ preferred), and a commitment to clinical excellence. Travel required within territory.

Qualifications

• Graduate of an accredited nursing program with current licensure to practice in Wisconsin.
• One year nursing experience required; three+ years preferred.
• Hospice or similar setting experience preferred.
• Knowledge of hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with insured vehicle.

Responsibilities

• Assess and monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans aligned with patients' wishes and goals.
• Administer medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on‑call rotation as required by local branch.
