Job summary

Generations Home and Hospice is seeking a Hospice RN Case Manager to deliver compassionate, patient-centered hospice care from our Fond du Lac area office. You will work within an interdisciplinary team to create individualized care plans, administer medications, and provide comprehensive pain and symptom management.

The role offers strong team support, reduced patient-to-nurse ratios, and a structured schedule (Mon–Fri, 8 AM–5 PM) with opportunities for professional growth.

Qualifications

• Graduate of an accredited nursing school with current licensure in the state of operation.
• One year of nursing experience required; 3+ years preferred.
• Experience in hospice or similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Must have a valid driver’s license and reliable vehicle.

Responsibilities

• Assess and monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient’s wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
