Job summary

Gentiva Hospice is seeking a Registered Nurse (RN) to provide compassionate, patient-centered care in Warrenton, Virginia. You will be the primary nurse guiding patients and families through the hospice journey, delivering hands-on nursing care and updating the Plan of Care at home or other care settings.

You will collaborate with physicians, LPNs, CNAs, social workers, and chaplains to create personalized care plans, assess patient conditions, document care, and educate families.

Qualifications

• Registered Nurse with current license.
• Hospice or palliative care experience preferred.
• Experience in home health or similar setting is a plus.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or facility settings.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized care plans and visit frequency.
• Coordinate with interdisciplinary team to refine care plans.
