Job summary

Gentiva Hospice in Virginia Beach seeks an RN Case Manager to lead compassionate end-of-life care at patients' homes or wherever they reside. You will perform assessments, manage symptoms, and educate families while coordinating with physicians, social workers, and care teams.

Qualifications

• Current RN license in the state of employment.
• CPR certification.
• One year RN experience in hospice, home health, or related settings preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments including in-person/telephonic RN assessments.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized plans of care and visit frequency.
• Coordinate care across interdisciplinary teams and consult with physicians.
