Job summary

Qualifications

• Currently licensed RN in the state of employment.
• Minimum 1 year RN experience (hospice, palliative care, acute care, or home health).
• CPR certification required.

Responsibilities

• Perform comprehensive RN assessments and reassessments, in person or via telephonic contact.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized plans of care and visit schedules.
• Coordinate care with physicians, LPNs, CNAs, social workers and chaplains.
• Educate patients and caregivers on disease, end-of-life care, medications, and home care.
