Hospice RN Case Manager — Lead Care & Family Support

Job summary

UnitedHealth Group is hiring a Registered Nurse Case Manager for Alabama Hospice Care. You will evaluate and manage the Plan of Care for hospice patients, collaborating with the IDG to implement treatment strategies promoting physical, psychosocial, emotional, and spiritual well-being.

The role requires an active RN license, a valid driver's license, and the ability to work in diverse home settings. Salary reflects local markets and full-time status.

Qualifications

• Current, unrestricted RN licensure in the state of practice.
• Current driver's license and access to a reliable vehicle or public transportation.
• Preferred CPR certification or ability to obtain within 90 days.

Responsibilities

• Makes initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identifies patient needs and re-assesses care every 15 days.
• Initiates and coordinates the plan of care with IDG members.
• Documents goals, interventions and patient/family responses.
• Collaborates with patient, family, physician, and care team daily.
• Instructs patient/family in self-care techniques.
• Maintains accurate clinical notes.
