Job summary

Gentiva is seeking a compassionate Hospice RN/Case Manager to deliver in-home nursing care and coordinate end-of-life services across patients' homes and facilities in Georgia.

You will perform initial assessments, develop individualized plans of care, and partner with physicians, social workers, and chaplains to ensure comfort, dignity, and quality of life for patients and their families. Strong communication and documentation skills are essential.

Qualifications

• Current license to practice nursing in the state of Georgia.
• Minimum 1 year RN experience in hospice, palliative care, or home health.
• CPR Certification and valid driver’s license with auto insurance.

Responsibilities

• Perform comprehensive patient assessments and reassessments including in-person/telephonic RN assessments.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized plans of care with visit frequency and risk identification.
• Coordinate care across interdisciplinary teams with physicians and other providers.
• Educate patients and caregivers on disease, self-care, end-of-life care, medications.
• Communicate changes in condition to the care team for timely interventions.
• Document timely clinical records and maintain infection control standards.
• Support IDG activities and care transitions as appropriate.
• Participate in on-call and patient support activities.
