Gentiva Hospice is seeking a Registered Nurse (RN) to provide comprehensive hospice and palliative care in Conyers, Georgia. The RN will assess patients, implement plans of care, and collaborate with physicians, CNAs, social workers, chaplains, and families to ensure compassionate end-of-life care.

Responsibilities include nursing interventions, patient and caregiver education, documentation, and adherence to regulatory standards.

Qualifications

• Requires Active RN license in Georgia.
• Hospice or palliative care experience preferred.
• One year RN experience in acute care, oncology, long-term care, hospice or home health.

Responsibilities

• Lead and coordinate patient care in home or facility settings.
• Document and monitor patient condition and plan of care.
• Collaborate with interdisciplinary team to refine care plans.
