Job summary

Gentiva seeks a dedicated Hospice RN/Case Manager to deliver personalized end-of-life care in patients' homes, assisted living, or other settings in the Savannah or Decatur areas. You will guide families through the hospice journey, perform comprehensive assessments, and document care plans with an interdisciplinary team.

The role emphasizes patient education, symptom management, and compassionate support for patients and loved ones, with limited after-hours on-call requirements in a full-time

Qualifications

• RN with hospice or home health experience preferred.
• Ability to educate patients and families about disease process and care.
• Experience coordinating with interdisciplinary teams and documenting care plans.

Responsibilities

• Be the primary point of contact for patients and families, guiding them through the hospice journey.
• Deliver skilled nursing care in home or other settings per Plan of Care.
• Perform initial and ongoing patient assessments and adjust care as needed.
• Collaborate with IDG to build and refine care plans.
• Provide emotional and bereavement support to patients and families.
