Job summary

Swan Hospice is actively seeking Registered Nurses (RN) to join our team in New Jersey. We provide quality hospice care to patients in skilled nursing facilities, assisted living facilities and in private homes.

The RN will oversee a primary caseload, perform assessments, develop and implement the plan of care with the IDT, and educate families and staff while ensuring compliance with state and federal regulations. A valid NJ RN license and transportation are required.

Qualifications

• Graduate of an accredited registered nursing program.
• Bachelor’s degree in Nursing preferred.
• Certified Hospice and Palliative Nurse (CHPN) certification preferred.
• Minimum of one year general nursing experience; hospice/home health/ med/surgical/critical care preferred.
• NJ RN license through NJ State Board of Nursing; valid driver’s license and reliable transportation.

Responsibilities

• Oversees primary caseload of Hospice patients and provides skilled professional nursing care.
• Performs initial and comprehensive nursing assessments and updates the care plan.
• Collaborates with the Interdisciplinary Team (IDT) to implement and revise the plan of care.
• Educates, supervises, and counsels patients, families, and staff on nursing care.
• Supervises Hospice Aides/Homemakers and ensures compliance with policies and procedures.
