Job summary

Swan Hospice in New Jersey is seeking Registered Nurses to oversee a primary caseload and provide skilled nursing care to patients, families, and caregivers as prescribed by the physician and in line with hospice standards.

The role includes initial and ongoing assessments, development and revision of the plan of care with the Interdisciplinary Team, teaching and supervising Hospice Aides, and coordinating care across facilities and homes.

Qualifications

• Graduate of an accredited registered nursing program.
• Bachelor’s degree in Nursing preferred.
• Certified Hospice and Palliative Nurse (CHPN) certification preferred.

Responsibilities

• Oversees primary caseload of Hospice patients and provides skilled nursing care as prescribed by the physician.
• Performs initial and comprehensive nursing assessments and identifies problems to develop the IDT plan of care.
• Reassesses and revises the plan of care to meet patient needs and collaborates with the IDT.
• Teaches, supervises, and counsels patients, families, and caregivers about nursing care and needs.
• Provides supervision to Hospice Aides/Homemakers and coordinates with other IDT members.
