Job summary

Longwood Hospice in the North Shore area is seeking an experienced Registered Nurse (RN) Case Manager to coordinate the patient and family/caregiver Plan of Care from referral to discharge. You will collaborate with the Interdisciplinary Team (IDT) and communicate changes in patient status with the attending physician and SNF partners.

The role requires an active MA RN license, solid computer skills, and excellent communication.

Qualifications

• Current RN license in good standing (MA).
• Competent with computers (MS Word, MS Excel).
• Strong verbal and written communication.
• Independent work experience.

Responsibilities

• Coordinate patient and family/c caregiver Plan of Care from referral to discharge.
• Facilitate communication about changes in patient status among IDT and attending physician.
• Plan, coordinate and deliver direct nursing care to patients and families as needed.
• Visit each patient at least every 14 days.
• Maintain a productive time per Hospice policy with direct care and care coordination.
