Job summary

Paylocity seeks an RN Case Manager to conduct admissions and provide nursing care in patients' homes. The role coordinates with the interdisciplinary team, patient and family, and referring agency to ensure comprehensive care.

The successful candidate will assess hospice patients, document the care plan, and maintain compliance with regulations while participating in ongoing education and QAPI initiatives.

Qualifications

• Two (2) years experience as a Registered Nurse in a clinical care setting and one (1) year full time hospice.
• Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
• Current State license as a Registered Nurse; Current driver’s license.
• Knowledge of state and federal regulations; good interpersonal skills.

Responsibilities

• Assess hospice patients utilizing criteria to identify physical, psycho-social, and environmental needs.
• Develop and document the plan of care in correlation with the assessment.
• Provide nursing care utilizing infection control measures.
• Collaborate with the Interdisciplinary Group/Team to meet patient/family needs.
• Communicate with attending physicians and hospice physicians involved in care.
• Submit all required paperwork in a timely manner.
• Participate in hospice in-service programs and QAPI program.
