Job summary

Longwood Hospice is seeking an Registered Nurse (RN) Case Manager to provide comprehensive nursing care and case management for hospice patients and families. You will coordinate the patient/family interdisciplinary plan of care and ensure regulatory compliance with the State Board of Nursing and Federal Conditions of Participation.

Qualifications

• Current license as a Registered Nurse (RN) in good standing with the MA State Board.
• Demonstrated computer competency (MS Word, MS Excel).
• Strong verbal, written, and interpersonal communication skills.
• Experience working independently with minimal supervision.
• Candidates must pass criminal background check and physical before orientation.

Responsibilities

• Coordinate the patient and family Plan of Care with IDT from referral to discharge.
• Communicate changes in patient status among IDT members and physician.
• Plan and deliver direct nursing care to patients and families.
• Visit each patient at least every 14 days as required.
• Maintain productivity standards and coordinate with facilities and staff.
• Supervise Home Health Aide Care Plans and document visits.
• Participate in regular Hospice IDT meetings and on-call rotation.
• Perform other duties as assigned.
