Gentiva is seeking a dedicated Hospice RN / RN Case Manager to lead comprehensive patient assessments in patients' homes or care settings. You will develop and adjust plans of care, coordinate with the IDG, and educate families to support quality end-of-life care with compassion.

Responsibilities include symptom management, documentation, and collaboration with physicians, LPNs, CNAs, social workers and chaplains to ensure smooth transitions and adherence to regulatory standards.

Qualificações

• Valid RN license in the state of employment.
• CPR certification required.
• Minimum 1 year RN experience in acute care, hospice, or home health.
• Hospice or palliative care experience preferred.

Responsabilidades

• Perform comprehensive patient assessments and reassessments, including in-person and telephonic RN assessments.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care, including visit frequency and risk identification.
• Coordinate care across interdisciplinary teams and consult with physicians as needed.
• Provide patient and caregiver education on disease process, self-care, end-of-life care, medications and nutrition.
