Job summary

aah in Illinois is seeking a full-time Hospice Case Manager to coordinate patient and family care across hospice settings, leading the interdisciplinary team to implement and update the plan of care for terminally ill patients. You will ensure comfort, dignity, and high-quality end-of-life care while navigating complex medical needs and family dynamics.

RN licensure and BLS certification are required. Expect 40 hours weekly (Mon–Fri, 8:00 AM–5:00 PM) with on-call for holidays; service areas

Qualifications

• RN license in the state of practice.
• BLS for Healthcare Providers certification.

Responsibilities

• Coordinate patient/family care to support terminally ill patients and families in hospice settings.
• Lead the IDG to develop, revise, and implement the Plan of Care with the interdisciplinary team.
• Enter and update documentation in the electronic health record per standards and regulations.
• Supervise Home Health Aide and licensed practical nurse visits.
• Facilitate discharge planning and ensure continuity of care after discharge.
• Assess patient response to treatments and adjust the care plan as needed.
• Communicate changes to physicians, supervisors and IDG members.
