Transitions Hospice in Chatham, IL is seeking an RN Case Manager to coordinate care for patients and families, align with the interdisciplinary team, and respond when it matters most.

You will conduct assessments, develop care plans, administer physician-ordered treatments, document accurately, educate families, and participate in on-call rotation. Must hold an active RN license in Illinois and have hospice or home health experience preferred.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.

Responsibilities

• Conduct comprehensive, ongoing assessments to understand each patient's evolving needs.
• Build and adjust personalized care plans with physicians and care teams.
• Administer medications and treatments per physician orders.
• Maintain accurate, timely clinical documentation.
