JourneyCare Hospice seeks a compassionate Hospice RN to join our team in Illinois. You will manage a patient and family caseload, performing assessments, admissions, and care planning to ensure high-quality end-of-life care.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR, First Aid Certification.
• Must be knowledgeable of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Manage patient and family caseload across assessment, eligibility, admission, planning, implementation and evaluation.
• Collect data from patient and family factors to tailor care with the team.
• Coordinate with attending physician and interdisciplinary team for optimal care.
• Provide regular home visits to reassess needs and update care plan.
• Perform clinical pronouncement of death and support the family.
• Maintain accurate patient records and timely care plans.
• Supervise and document delivery of Home Health Aide care per guidelines.
