JourneyCare Hospice in McHenry, IL is seeking an experienced Registered Nurse Case Manager to join our team. The Hospice RN will manage a patient and family caseload, coordinate admission, assessment, planning, implementation, and evaluation of care, and work with an interdisciplinary team to deliver compassionate end-of-life services.

Requirements include RN licensure in IL, CPR and First Aid, and a driver’s license.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• CPR and First Aid certifications required.
• Knowledge of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance required.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission processing, planning, implementation, and evaluation of the nursing process.
• Obtain data from patient physical, psychological, social, and spiritual factors and initiate intervention by the interdisciplinary team.
• Coordinate care by communicating with the attending physician and other agencies.
• Provide regular visits to reassess patient needs and update the plan of care.
• Maintain accurate patient records, charts, and care plans timely.
• Supervise and document delivery of home health care as required by guidelines.
