Job summary

Transitions Hospice in Williamsville, IL seeks a Registered Nurse Case Manager to coordinate end-of-life care and lead the interdisciplinary team. This full-time role covers Sangamon, Morgan, Cass, Menard, Mason counties with 8am–8pm shifts, rotating 4/3 day weeks including weekends.

Responsibilities include ongoing patient assessments, care plan development with physicians, medication administration, documentation, and family education.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• One (1) + year Hospice, home health or related organization experience preferred.
• Compassion for patients and families navigating end-of-life care
• Strong clinical judgment and the ability to work independently in the field
• Excellent communication skills across care teams and families

Responsibilities

• Conducting comprehensive, ongoing assessments to understand each patient's evolving needs
• Building and adjusting personalized care plans in collaboration with physicians and care teams
• Administering medications and treatments per physician orders
• Keeping clinical documentation accurate, timely, and meaningful
• Serving as the communication bridge between families, physicians, and community health partners
• Educating patients and caregivers so they feel confident and supported
• Participating in on-call rotation per team policy
