Job summary

Advocate Health in Milwaukee South is seeking a dedicated Hospice Case Manager to coordinate patient and family care across hospice settings. Responsibilities

• Identify and coordinate patient/family care to support terminally ill patients and families in hospice settings.
• Educate patients and families about disease processes and hospice care.
• Review caseload to ensure appropriate level of care and referrals as needed.
• Lead the IDG, develop and implement the Plan of Care with interdisciplinary team.
• Enter and update documentation in the EHR per standards.
• Monitor patient response to treatments and revise care plans accordingly.
• Supervise Home Health Aide and licensed practical nurse visits.
• Coordinate discharge planning and community resource utilization.
• May conduct OASIS assessments for Palliative Care patients in WI (on occasion).
