Job summary

Allegiance Healthcare - Northeast Hospice in West Monroe, LA seeks a registered nurse to coordinate patient-specific POC implementation. The RN will document patient status, collaborate with the IDT, and guide patient/family self-care practices.

The role includes supervising aides, ensuring accurate clinical notes within one week, and participating in on-call rotations. Positions available: full-time, part-time, PRN.

Qualifications

• Licensed RN currently in Louisiana with no restrictions.
• May substitute two years of full-time LPN experience for RN requirements.
• Must be an employee of the hospice and coordinate with all employers.

Responsibilities

• Coordinate implementation of the POC for each patient.
• Document problems, goals, interventions, and responses.
• Collaborate with attending physician and IDT members.
• Educate patient/family in self-care techniques when appropriate.
• Supervise hospice aides and delegate duties as needed.
• Submit annual and supervisory documentation for aides and LPNs.
• Participate in on-call rotations.
