Job summary

Allegiance Healthcare West Central Hospice Leesville, LA is seeking an experienced registered nurse with management experience to lead patient care operations and coordinate hospice services. This role requires active RN licensure in the state, a minimum of two years RN experience, and prior hospice/home care or supervisory background.

Proficiency with Microsoft Office and/or Google Suite is preferred; field visits and on-call rotation are involved.

Qualifications

Responsibilities

• Review patient information related to the case and home visits to determine hospice needs.
• Assign hospice personnel to cases and participate in case conferences with Medical Director.
• Maintain documentation accurately in the electronic health record.
• Assist in planning, implementation, and evaluation of continuing education for staff.
• Promote quality service and customer satisfaction within the organization.
