Job summary

Allegiance Healthcare Hospice and Palliative Care in West Monroe, LA, seeks a registered nurse to coordinate the implementation of the POC for each patient. Responsibilities include assessing physical, psychosocial, and environmental needs, coordinating with the IDT, and ensuring documentation within the clinical record.

The RN will supervise aides, provide patient/family education, and participate in on-call rotations.

Qualifications

• Licensed registered nurse with no state restrictions in Louisiana.
• Two years of full-time RN experience required; hospice/ LPN experience may substitute.
• Employee of the hospice and coordinate duties if employed by multiple agencies.

Responsibilities

• Provide nursing services per the POC to patients.
• Document issues, goals, interventions and patient/family response in the chart.
• Collaborate with attending physician and IDT for patient/family care.
