Job summary

Allegiance Healthcare South Central Hospice in Lafayette, LA seeks a registered nurse to coordinate the patient care plan (POC) and lead the hospice team in delivering high-quality end-of-life care.

Responsibilities include assessing patient needs, documenting care, supervising aides, and ensuring timely clinical notes and adherence to the POC. RN licensure in Louisiana required, with two years of RN experience (hospice experience preferred).

Qualifications

• Licensed registered nurse in Louisiana with no restrictions.
• Two years full-time RN experience; two years hospice bedside RN experience may substitute.
• Must be an employee of the hospice and coordinate duties with other agencies if applicable.

Responsibilities

• Identify patient/family needs per POC and reassess at least every 14 days.
• Provide nursing services in accordance with the POC.
• Document problems, goals, interventions, and patient/family response.
• Collaborate with attending physician and IDT to deliver care.
• Instruct patient/family in self-care techniques when appropriate.
• Supervise ancillary staff and delegate duties.
• Submit accurate clinical notes within one week of visits.
• Supervise and evaluate hospice aide performance and relationships.
• Conduct supervisory visits to patient residences at least every 14 days.
• Documentation of supervision and annual performance reviews.
• Participate in on-call rotation as needed.
