Job summary

Choice Hospice, Inc. in Greenwood Village, CO seeks a dedicated Hospice RN Case Manager to coordinate patient-centered care in home and community settings. The RN will lead assessments, plan and monitor care, and collaborate with a multi-disciplinary team to ensure comfort and dignity for patients at the end of life.

The role emphasizes education for patients and families, on-call coverage, and close communication with physicians. A valid Colorado RN license and CPR are required.

Qualifications

• Active RN license in Colorado and CPR certification.
• Valid driver's license and auto insurance.
• At least 2 years of hospice RN experience preferred.
• HomeCare HomeBase experience preferred.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patient needs and document findings.
• Develop and implement the plan of care with the IDG for quality and continuity.
• Educate patients and families about disease, end-of-life care, and self-care.
• Provide clinical directions to aides and LPNs, and coordinate care.
• Prepare progress notes and communicate changes to the physician.
• Carry on-call responsibilities as assigned.
