Job summary

Horizon Hospice of Colorado is seeking a skilled Registered Nurse Case Manager to coordinate and deliver hospice care across the Denver Metro area. You will conduct comprehensive nursing assessments, lead an interdisciplinary team, and provide direct patient visits in homes and care settings.

This full-time role starts at 0.8 FTE (32 hours per week) with growth to 1.0 FTE (40 hours). Expect a typical caseload of 12-15 patients and travel within the service area, with mileage reimbursement

Qualifications

• Current unrestricted RN license in Colorado.
• Minimum of one year hospice nursing experience.
• Strong assessment and symptom-management skills.
• Ability to work independently in community settings with an IDT.

Responsibilities

• Perform comprehensive nursing assessments and ongoing reassessments.
• Develop and update individualized hospice plans of care with IDT.
• Provide skilled visits in homes, assisted living, SNFs, and other settings.
• Manage pain and symptoms; administer medications and interventions.
• Educate patients and caregivers on medications, progression, and end-of-life changes.
• Coordinate medications, equipment, and services to support the plan of care.
• Document timely in the electronic medical record; ensure HIPAA compliance.
