Job summary

Solace Hospice & Palliative Care Inc seeks a compassionate Hospice RN Case Manager for Saint Charles, IL. You will lead end-of-life care, guide patients and families, and collaborate with a multidisciplinary team to ensure dignity and comfort.

Responsibilities include home visits, care planning, IDT coordination, and on-call coverage. The role requires strong clinical skills, empathy, and independent work in a dynamic hospice setting.

Qualifications

• Active IL RN license in good standing.
• 1-2 years nursing experience; hospice/palliative/home health preferred.
• Strong knowledge of Medicare hospice regulations and care standards.
• Excellent clinical assessment, pain management, and symptom control.
• Exceptional communication and interpersonal skills with patients, families, and care teams.
• Ability to work independently; manage a dynamic caseload.
• Valid driver’s license and reliable transportation for field visits.

Responsibilities

• Manage a caseload of hospice patients; assessments and care plans.
• Perform skilled nursing visits in patients' homes, SNFs, and other settings in the Saint Charles area.
• Coordinate interdisciplinary team care with physicians, social workers, chaplains, and aides.
• Attend every-other-week IDT meetings (in person or via phone/video) to review plans.
• Monitor and manage pain and symptoms; adjust care plans as needed.
• Educate patients and families on disease progression, comfort care, and end-of-life planning.
• Maintain accurate documentation compliant with hospice regulations and Medicare guidelines.
• Participate in on-call rotation for after-hours and weekend coverage.
