Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to lead patient care in home and community settings. You will perform comprehensive assessments, provide symptom management, and coordinate with an interdisciplinary team to implement personalized plans of care.

Requirements include RN licensure and CPR, with at least 1 year of hospice/home health experience preferred. Travel to patient homes is expected, with full-time benefits and long-term growth opportunities.

Qualifications

• RN with active license in state of employment.
• Experience in hospice, palliative care, home health preferred.
• CPR certification required and valid driver’s license.

Responsibilities

• Perform comprehensive in-person and telephonic RN assessments and education.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care with visit frequency.
• Coordinate care with IDG and physicians.
• Educate patients and caregivers on disease, end-of-life care, medications and nutrition.
