Gentiva is seeking a dedicated Hospice RN / RN Case Manager to deliver hands-on nursing care in patients' homes, assisted living, or other residence settings. Responsabilités

• Perform comprehensive patient assessments and reassessments in home or facility settings.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care including visit frequency and risk identification.
• Coordinate care across IDG including physicians and other healthcare professionals.
• Educate patients and caregivers on disease processes, end-of-life care, medications, nutrition and self-care.
• Communicate changes in condition to the care team for timely interventions.
• Support care continuity by coordinating home health, palliative, and hospice referrals.
• Document timely and accurate clinical records and ensure regulatory compliance.
• Participate in on-call and patient support activities as applicable.
