Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to lead patient care in home settings, guided by individualized plans of care. You will collaborate with an interdisciplinary team to deliver compassionate, evidence-based hospice services and education for patients and families.

The role emphasizes comprehensive assessments, symptom management, and coordination of care across settings, with a focus on quality, compliance, and patient-centered support.

Qualifications

• RN licensed to practice in the state of employment.
• CPR certification.
• Current automobile insurance and valid driver’s license.

Responsibilities

• Perform comprehensive patient assessments and reassessments including in-person/telephonic RN assessments and provide education to patients, family members and caregivers.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care including patient visit frequency to meet needs and risk.
• Coordinate care across interdisciplinary teams including physicians.
• Provide patient and caregiver education regarding disease process, self-care, end-of-life care, medications and nutrition.
• Document timely clinical records on all patients and maintain compliance with infection control standards.
