Job summary

Gentiva Hospice in the United States seeks an experienced RN Case Manager to deliver in-home hospice care, perform comprehensive assessments, and develop personalized plans of care. You will coordinate with physicians, LPNs, CNAs, social workers, chaplains, and other team members to ensure compassionate, patient-centered care.

Responsibilities include educating families, documenting care, ensuring infection control, and supporting end-of-life discussions.

Qualifications

• Must be a Registered Nurse with current license.
• Excellent patient assessment skills and clinical documentation.
• Experience in hospice, palliative care, or home health preferred.
• Ability to coordinate care across an interdisciplinary team.

Responsibilities

• Perform comprehensive RN assessments and ongoing re-assessments.
• Develop and update individualized plans of care and visit frequency.
• Coordinate care with physicians and other IDG members.
• Educate patients, families and caregivers on disease process, medications, and end-of-life care.
• Document clinical records timely and maintain infection control standards.
