Job summary

Mission Healthcare is seeking a Registered Nurse to join our home health team serving San Diego County. You will perform initial patient assessments, develop and adjust care plans, and re-evaluate patient needs to ensure quality home health services.

The ideal candidate holds an active RN license, BLS CPR certification, and at least 1 year of acute care experience; a BSN is preferred. This full-time role offers a comprehensive benefits package and a $2,500 sign-on bonus.

Qualifications

• Current and valid state RN license is required.
• Basic Life Support (BLS) certification is required.
• Bachelor’s degree in Nursing and 1 year of home health experience preferred.

Responsibilities

• Completes an initial assessment of the patient and family to determine home care needs.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Regularly re-evaluates patients’ needs.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan which establishes goals, based on nursing diagnosis, incorporating therapeutic, preventive, and rehabilitative nursing actions.
• Includes the patient and family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
• Counsels the patient and family in meeting plan of care and related needs.
• Provides health care instructions to the patient as appropriate per assessment and plan.
• Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
