Job summary

Silverado Hospice in the San Diego area is hiring an RN Case Manager to coordinate care and develop individualized plans for patients facing life-limiting illnesses. You’ll deliver hands-on hospice and palliative care, working with families and the interdisciplinary team to ensure quality outcomes.

The role requires an active California RN license, at least 1 year of RN experience (hospice preferred), and the ability to lift up to 50 pounds.

Qualifications

• Must hold a valid RN license in California.
• Minimum 1 year of RN experience; hospice experience preferred.
• Must be able to lift up to 50 pounds and possess a valid driver’s license.

Responsibilities

• Develop detailed Plans of Care for assigned patients, coordinating with the Interdisciplinary Group.
• Provide skilled hospice and palliative care for patients and families.
• Conduct comprehensive nursing evaluations and assessments; supervise LVNs/Home Health Aides.
