Job summary

Mission Healthcare is seeking a licensed RN for a full-time home health role serving San Diego County. The nurse will perform initial assessments, plan of care, and ongoing evaluations to support patient independence and comfort at home.

Responsibilities include diagnosing and revising care plans, administering prescribed treatments, counseling patients, and coordinating discharge planning. Requires an active RN license, BLS, and at least one year in acute care or home health preferred.

Qualifications

• Current and valid state RN license.
• Minimum of 1 year of recent acute care experience in an institutional setting.
• Basic Life Support (BLS), with hands‑on CPR certification.
• Bachelor’s degree in Nursing, and 1 year of home health experience preferred.

Responsibilities

• Completes an initial assessment of the patient and family to determine home care needs.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Regularly re-evaluates patients’ needs.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan which establishes goals, based on nursing diagnosis, incorporates therapeutic, preventive, and rehabilitative nursing actions.
• Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed by the physician.
• Counsels the patient and family in meeting plan of care and related needs.
• Provides health care instructions to the patient as appropriate per assessment and plan.
• Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
