Zusammenfassung

Mission Healthcare in San Diego County is seeking an experienced Registered Nurse for full-time home health visits. You will perform initial assessments, develop and revise care plans, and coordinate patient care with families and physicians.

The ideal candidate holds an active RN license, a year of acute care experience, and BLS certification. We offer a competitive pay range of $55–$60 per hour, a $2,500 sign-on bonus, and comprehensive benefits package.

Qualifikationen

• Current and valid state RN license.
• Minimum of 1 year of recent acute care experience in an institutional setting.
• BLS with CPR certification.
• Bachelor’s degree in Nursing; 1 year of home health experience preferred.

Aufgaben

• Performs initial patient assessment and family evaluation to determine home care needs.
• Develops and revises the plan of care as patient status changes.
• Regularly re-evaluates patient needs and updates nursing diagnoses.
• Administers medications and treatments as prescribed by the physician.
• Counsels patient and family and provides care instructions.
• Identifies discharge planning needs and prepares for discharge.
