Job summary

Mission Healthcare, a leading home health and hospice provider in the western United States, seeks a registered nurse to perform initial assessments, develop and revise care plans, and coordinate patient care in Carson City and surrounding areas.

You will administer medications per physician orders, educate patients and families, and collaborate with multidisciplinary teams to deliver compassionate, quality care.

Qualifications

• Current and valid state RN license.
• BLS and CPR certification required.
• Experience in home health preferred; strong care planning and patient education skills.

Responsibilities

• Completes an initial assessment of the patient and family to determine home care needs.
• Initiates the plan of care and revises it as patient status changes.
• Regularly re-evaluates patients’ needs and documents nursing diagnoses.
• Develops a care plan with goals based on nursing diagnosis; involves patient and family.
• Administers medications and treatments as prescribed by the physician.
• Counsels patient and family on plan of care and related needs.
• Provides health care instructions per assessment and plan.
• Identifies discharge planning needs and coordinates prior to discharge.
