Job summary

Elara Caring is seeking a Registered Nurse Case Manager, Hospice in Mentor, Ohio to join our team delivering compassionate end-of-life care across home-based settings.

The RN Case Manager will assess needs, coordinate care, educate patients and families, and support staff with a focus on hospice philosophy and pain management. Valid Ohio RN license required; 1+ year clinical experience; valid driver’s license and reliable transportation to travel between patient homes.

Qualifications

• Current State license as a Registered Nurse (RN).
• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.

Responsibilities

• Assess hospice patients and families to identify needs across physical, psychological, social, spiritual, and environmental domains.
• Monitor patient condition and promptly communicate significant changes to the physician and care team.
• Educate and support patients, families, caregivers, and staff in following the interdisciplinary Plan of Care.
