Job summary

Elara Caring is seeking a Registered Nurse Case Manager, Hospice in Willoughby, Ohio. The RN will assess patient and family needs across physical, psychological, social, spiritual, and environmental domains, monitor changes, and educate on the care plan and hospice philosophy.

The role requires an active RN license, 1+ year of clinical nursing experience, and the ability to travel between homes with reliable transportation. Hospice or home health experience is preferred.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited School of Nursing (Certificate, Diploma, Associate or Baccalaureate).
• 1+ year of nursing experience in a clinical care setting.
• Ability to lift up to 50 pounds and travel between patient homes.
• Driver's license or reliable transportation for travel to locations.
• Hospice or home health experience is preferred.

Responsibilities

• Assess hospice patients and families to identify needs across physical, psychological, social, spiritual, and environmental areas.
• Monitor patient condition and report significant changes to physician and care team.
• Educate and support patients, families, caregivers, and staff in the interdisciplinary care plan.
• Provide education on hospice philosophy, pain and symptom management to staff in various care settings.
• Provide compassionate support at end of life and complete bereavement assessments for families.
