Job summary

Elara Caring is seeking a Registered Nurse Case Manager to join the Lake or Cuyahoda County territory, providing expert hospice and home health services across patient homes and care facilities. You will assess needs, educate families, and coordinate with the care team to deliver compassionate, timely care.

The role requires an active RN license, nursing degree, 1+ year clinical experience, ability to lift up to 50 pounds, and valid driver’s license.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited nursing program (Certificate, Diploma, Associate, or Baccalaureate).
• 1+ year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds and travel between patient homes.
• Hospice or home health experience preferred.

Responsibilities

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and communicate significant changes to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in following the Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff in various care settings.
• Provide compassionate support at the time of death and complete bereavement assessments.
