Job summary

Heartland Hospice is seeking a dedicated Hospice RN / RN Case Manager to lead in-home patient care across Ohio. You will perform comprehensive assessments, develop plans of care, and coordinate with physicians and the IDG to ensure quality end-of-life services.

The role emphasizes family education, symptom management, and compassionate communication, with flexible scheduling and full nursing responsibilities in patients’ homes and community settings.

Qualifications

• Current license to practice nursing in the state of employment.
• CPR Certification.
• Current automobile insurance and valid driver’s license.
• Hospice or palliative care experience preferred.
• One year RN experience in related settings.

Responsibilities

• Perform comprehensive patient assessments and reassessments including in-person and telephonic RN assessments.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care including visit frequency.
• Coordinate care across interdisciplinary teams and physicians.
• Educate patients, families, and caregivers about disease processes and self-care.
