Job summary

Elara Caring is seeking a Registered Nurse Case Manager to continue our industry leadership in hospice and home health. You will assess patients and families, coordinate care, and educate staff across nursing homes, hospitals, and other settings to ensure the Plan of Care is followed.

You will travel to patient homes delivering Right Care at the Right Time in the Right Place. The role requires an active RN license, a nursing degree, 1+ year of clinical nursing experience, excellent

Qualifications

Current State license as a Registered Nurse.
Graduate of an accredited nursing program (Certificate, Diploma, ADN, or BSN).
1+ year of nursing experience in a clinical setting.
Ability to lift up to 50 pounds and transfer patients.
Valid driver's license and reliable transportation for home visits.

Responsibilities

Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
Monitor patient condition and communicate changes to physician and care team.
Educate and support patients, families, caregivers, and facility staff in the Plan of Care.
Provide education on hospice philosophy, pain and symptom management to staff in care settings.
Provide compassionate support at time of death and complete bereavement assessments.
