Job summary

Salus Hospice in Orange County is seeking a compassionate Hospice RN Case Manager to join our dedicated team serving Central and Southern Orange County. You will work with the Interdisciplinary Team to assess patient needs and coordinate hospice services in various settings.

The ideal candidate holds a CA RN license, has hospice experience, and demonstrates excellent interpersonal skills, reliability, and a patient-centered approach to end-of-life care.

Qualifications

• Registered Nurse with CA state license.
• Minimum one year of hospice RN experience.
• Strong background in medical/surgical, ICU/CCU, or ER is helpful.
• Valid driver's license, auto insurance, and reliable transportation.
• Excellent interpersonal skills and passion for hospice care.
• Understanding of hospice philosophy and terminal illness needs.
• Self-directed with ability to work with little supervision.
• Current physical and TB test results.

Responsibilities

• Perform RN duties on hospice team.
• Serve as an RN Case Manager for the hospice Interdisciplinary Team.
• Assess for signs and symptoms of discomfort and take prompt action.
• Establish plan of care while collaborating with the Interdisciplinary Team including physicians when appropriate.
• Coordinate all hospice services to patients/families in all settings to ensure quality of care.
